Researchers collected multiple blood samples and measured how smokers puffed on an e-cigarette with different flavors. They found that when smokers puff on fruit-flavored e-cigarette, they took longer and deeper puffs compared to tobacco cigarettes. Vaping different flavors resulted in varying levels of plasma nicotine.

Interestingly, when smokers used a fruit-flavored e-cigarette (cherry), they inhaled higher doses of nicotine compared to other flavors. This study suggests that flavors in e-cigarettes may change the way smokers puff on the product, and this may result in inhalation of higher nicotine doses.

A wide variety of non-cigarette forms of tobacco and nicotine exists and their use varies regionally and globally. Smoked forms of tobacco such as cigars, bidis, kreteks, and waterpipes have high popularity and are often perceived erroneously as less hazardous than cigarettes, when in fact their health burden is similar. Smokeless tobacco products vary widely around the world in both form and health hazards, with some clearly toxic forms (e.g. South Asia), and some forms with far fewer hazards (e.g., Sweden). There are also emerging nicotine delivery systems not directly reliant on tobacco (e.g. electronic nicotine delivery systems [ENDS]). The presence of such products presents both challenges and opportunities for public health. Future regulatory actions such as expansion of smokefree environments, product health warnings, and taxation may serve to increase or decrease the use of non-cigarette forms of tobacco. These regulations may also bring about changes in non-cigarette tobacco products themselves that could impact public health by affecting attractiveness and/or toxicity.

Findings from Roswell Park study suggest benefit from standardization, regulation of products

According to Maciej Goniewicz, PhD, PharmD, both flavorings and high voltage increase the level of toxicity found in e-cigarettes.

BUFFALO, N.Y. — Roswell Park Comprehensive Cancer Center scientists report that several flavorings added to electronic cigarettes impact the toxicity of the devices and that, among the tested flavors, strawberry was the most toxic. The researchers also confirmed an earlier finding that increasing the battery output voltage of these devices significantly increases toxicity. The study has been published online ahead of print in the journal Tobacco Control.

Although many of the flavorings used in e-cigarette liquids have been certified as safe for eating, little is known about their effects when heated and inhaled in e-cigarettes,” says the study’s senior author, Maciej Goniewicz, PhD, PharmD, Assistant Professor of Oncology in the Department of Health Behavior at Roswell Park. “This study suggests that various characteristics of e-cigarettes, including any flavorings, may induce inhalation toxicity and therefore, caution should be used with these products until more comprehensive studies are performed.”
